Submitting your Wellness Program activity documentation:

There are two types of wellness activities:

1) those that require documentation from the employee 
2) those that use attendance rosters submitted by a designated representative.

When submitting receipt or other documentation, you must include THE FORM (linked below).


THE FORM

Instructions

1)  Attach your documentation of activity completed to THE FORM     

2)  Scan and e-mail THE FORM and documentation to wellness@nic.edu as an attachment or submit via campus mail to HR.

3) Documents received by the last business day of the month will be credited to your account no later than the 15th of the following month.

4) Check your HRA balance to verify incentives have been credited to your account (instructions to check your HRA are below via the Peak1 Administration link).
Note:
There are some wellness activities that do not require THE FORM, as they are attendance based and attendance rosters will be submitted to HR by a designated representative.

BioIQ

This is the website where your primary dependent (18 or older) can schedule their biometric screenings. Employees can also use this link if they have not already logged in through their MYNIC account.

NEW CODE: NIC2013

 Regence BlueShield website

This is the website where you complete your General Health Assessment. 

Note:
For the lower premium for benefits that begin July 1, 2011, the General Health Assessment must have been completed by
May 20, 2011.

Login Instructions

This is the website where you log in to see your account balance for the Wellness Program incentives (HRA). 

If you are contributing to your Flexible Spending Account (FSA), which is separate from the Welpness Program account, you will also see your balance for that account on this website.
